NEHR (Singapore National Electronic Health Record) is the unified patient health record shared across Singapore's healthcare system. Public hospitals, polyclinics, and contributing private providers share key clinical data — visits, diagnoses, medications, lab results, imaging reports — through NEHR. The patient's record follows them across providers, reducing duplicate testing and improving care continuity.
NEHR is operated by Synapxe (the renamed IHiS, the national health technology agency) under the Singapore Ministry of Health. Participation by public providers is mandatory; private-sector contribution is increasing under the Healthier SG framework. Patient access to NEHR data is via HealthHub, the national patient portal.
