Social prescribing is the practice of clinicians referring patients to community-based, non-medical resources to address health needs rooted in social determinants. Resources include exercise classes, support groups, food assistance programmes, employment services, debt advice, housing assistance, arts and culture programmes. Pioneered in UK NHS where every primary care network has access to a Social Prescribing Link Worker.
The rationale: many patient health issues are rooted in social context (loneliness, food insecurity, financial stress, lack of physical activity, social isolation) that medical care alone cannot fully address. Connecting patients to community resources addresses these root causes.
